Environment
Description
In IEEE version of 1364-2001, there is no Index.
That is usual IEEE policy.

But IEEE Standards Style Manual says we can request an
Index. Need to work together with IEEE Standards Project
Editor. Index must refer to subclause number rather than
page number.
Fix
Return Index.

Work together with IEEE Editor.

Index must refer to Subclause #, not page #.

Note: there may not be time to add index references to text
added in 2001c.

So far, we have only done corrections and minor additions to the 1364-2001 text.

But if we start adding more major changes, like SystemVerilog stuff,
then it is a whole different ballgame, requiring a LOT more work.
