Need to check everywhere that constant_expression is used in BNF, that it
really does need to obey all the restrictions of constant_expressions,
such as no hierarchical references, etc.

Need to distinguish in text between constant expressions which have such
restrictions and those which do not.

As an additional example of how careful we have to be about the term "constant
expression",
12.2.1, para. 2, says that

"The expression on the right-hand side of the defparam assignments shall be a
constant expression involving only numbers and references to parameters."

But it fails to note that constant function calls are also allowed.

In addition, the text at the beginning of Section 4 which defines the term
"constant expression" should explicitly list all its restrictions, such as
no hierarchical references.
